Who are defined immediate family members?

The insured person’s spouse, parent or child - the same family members covered under FMLA.

If my infant gets sick and needs extended care, am I eligible to submit a claim? 

Yes. If you are an insured person past the benefit waiting period and the sickness is a serious health condition, then you may submit a claim.

Are there different benefit options offered?

We offer three options of coverage to employers to choose from: 1) an election by the policyholder for all eligible employees; 2) a voluntary election by the eligible employee, or a combination of 1 and 2. The employer can select plan designs replacing as little as 40% and up to a maximum of 80% of your weekly earnings subject to a maximum weekly benefit of $1,500.

How is CFL’s Voluntary coverage paid? 

An eligible person completes an enrollment form with information required.  If the eligible person is required to pay all or part of the premium for coverage, then s/he must acknowledge his or her permission for the policyholder to withhold premium payments from wages.

What is the maximum benefit period?

10 weeks during any 52-week period.

Can the FMLA form WH-380-F be used for medical certification when making a claim?

 Yes, it can.
